Capture mouse input while a mouse button is being pressed down so that we get a release event even if the mouse pointer is outside the window. Also get dx and dy in that case.

Made it so the D3D driver for the primitives addon uses FVF vertices by default, removing the last state from that driver and hopefully making the whole addon thread safe. Note that this means that the addon now officially supports 3D vertices, even though the software component can't draw them yet.

Removed the indexed primitives hack, and the alloca usage from the primitives...
