Fix a number of issues found through static analysis from cppcheck. Extra breaks, uninitialized vars in constructors and limiting scope of variables. No functionality change intended.

GLES2: Fix several of the sample shaders that need rotation matrices. It is a column vs row major issue, which GLSL ES is the former.

Patch 3144242 - Clarify Compositor clear pass syntax. Extra "clear" token is no longer required. Now more consistent with docs and other pass types.
